Both tenses translate to English 'was/were' or a simple past, which is exactly why English speakers struggle here. The real test isn't the translation — it's the question: is this the background, or is this the thing that happened?
Mentre camminavo, ho visto un gatto.
camminavo sets the ongoing scene; ho visto is the single thing that interrupted it.
A simple test
- Can you put a clear start/end on it? → passato prossimo
- Was it ongoing, repeated, or descriptive? → imperfetto
- Da bambino, giocavo sempre — 'as a child, I always used to play' (habit) → imperfetto
